Purpose. The therapist or stylist completes this form with the client before any chemical, skin penetration, peel, needling or light-based service.

1.When to use this form

Use this form for every new client and for every existing client before a service with a chemical, heat, skin penetration or light-based component. The form must be completed on the day of treatment and signed by the client before the service begins. Scan the completed form to the client record and shred the paper copy.

2.Completing the health screen

Ask each screening question in plain language and record the answer given. Do not skip questions because the client is a regular. Where an answer indicates a contraindication, stop and consult the site leader before proceeding.

3.Explaining the service

Describe what will happen, how it will feel, how long it will take, the likely results and the aftercare. Explain the risks in words the client understands. Invite questions and record any that the client raises.

4.Recording consent

Read the consent statement aloud or have the client read it. The client must sign and date the form. The staff member must countersign. For clients under 18 the parent or guardian must sign.

5.After the service

Record the products, settings and any reaction observed in the treatment notes on the client card. Give the client written aftercare and note that it was provided.

6.Complete the form
