Purpose. Therapists and colour specialists use this template to record a multi-visit plan for a client, including products, timing, home care and review points.

1.Purpose of the plan

A treatment plan turns the consultation into a sequence of services that the client understands and agrees to. It sets expectations about results and cost, and it records the decisions so that another team member can continue the plan. Use it for colour corrections, chemical reformation programs, skin programs of three or more visits and pre-event hair or skin preparation.

2.Assessment summary

Summarise the findings from the consultation: hair or skin condition, history, contraindications and the client's goal in their own words. Attach or reference the consent form and the patch test result.

3.Planned services

List each visit in order with the service, products and settings, expected duration and price. State the interval between visits and any home care products the client needs to use between them.

4.Home care and aftercare

Record the retail products recommended and the instructions given. Be specific about what the client must avoid, such as heat styling, sun exposure or active skincare ingredients, and for how long.

5.Review and sign-off

Set a review point after which the plan will be reassessed. The client and the staff member sign the plan and a copy is saved to the client record.
