Client Consultation and Consent Policy

Purpose. This policy requires a recorded consultation and informed consent before any chemical, heat, skin penetration or light-based service is delivered.
1.Why consultation matters
A consultation identifies what the client wants, whether the service is suitable, and what risks apply to that person on that day. It protects the client from harm and the business from complaints and claims. Every service at Luxe starts with a consultation, and the depth of the consultation must match the risk of the service.
2.Consultation requirements
Stylists, barbers and therapists must complete the consultation section of the client card before the service starts. For chemical, skin and light-based services the consultation must cover contraindications, medications, allergies, previous reactions, recent treatments and sun exposure. Where a contraindication is present the service must not go ahead until it has been cleared by the site leader or, where required, by the client's medical practitioner.

- Ask about allergies, medications and skin conditions
- Check previous services and any reactions
- Agree the service, expected outcome, time and price
- Record the plan on the client card
3.Patch testing
A patch test must be completed at least 48 hours before the first use of any oxidative hair colour, lash or brow tint, or lash adhesive on a new client, and repeated when the product changes or the client reports a reaction anywhere. The result must be recorded on the client card. A client who declines the test will be offered non-oxidative alternatives only.
4.Informed consent
For skin penetration, peels, needling, light-based treatments, cosmetic tattooing and chemical reformation the client must sign the Client Consultation and Consent Form after the therapist has explained the procedure, the likely sensations, the risks, the aftercare and the alternatives. Consent must be obtained on the day of treatment and must not be assumed from a previous visit.
5.Clients under 18
A parent or guardian must consent in writing for any chemical, skin penetration or light-based service for a client under 18 years of age. Clients under 16 must not receive skin penetration services. Staff must check identification where age is unclear and must record the guardian's name on the client card.
6.Records and privacy
Consultation records and consent forms must be kept for seven years in the client record system and must be handled under the Privacy and Client Records Policy. Only staff involved in the client's care and the site leader will access them.
